Why do my QuickTime streams not play?

The main problem that people have when streaming QuickTime files is that the files have been encoded for a progressive download instead of in the correct streaming format. This results in the QuickTime player stating that the file is not found (404 error) even though there is a legitimate progressive download file in its place.
